Existing Bomaid members must register their new-born infants as dependents within 48 hours of birth to ensure immediate cover. To register your new-born, you will need to supply the following information:

• Bomaid membership details for the main member i.e. membership number
• Birth notification provided by the hospital, including full names of the baby, date of birth and gender

Application for Additional Dependent Form

Single male applicants who wish to register a child born out of wedlock must complete an application form and supply the following additional information for their child:

• Application form
• Abridged birth certificate
• Sworn Affidavit* (*only required if the father’s name does not appear on the birth certificate)
