How to Become a Member

For information on how to become a member, select the section that applies to you; follow the instructions and download the forms if necessary.

Submit your completed forms to Bomaid.

Applicants will need to submit the following:

  • Individual Application for Membership (completed by main member applicant)
  • Company/Corporate Application

If you are applying through your employer, please ensure your forms have been authorised by a company representative before submitting them to Bomaid.

Existing Bomaid members must register their new-born infants as dependents within 48 hours of birth to ensure immediate cover. To register your new-born, you will need to supply the following information:

  • Bomaid membership details for the main member i.e. membership number
  • Birth notification provided by the hospital, including full names of the baby, date of birth and gender

Single male applicants who wish to register a child born out of wedlock must complete an application form and supply the following additional information for their child:

  • Application form
  • Abridged birth certificate
  • Sworn Affidavit* (*only required if the father’s name does not appear on the birth certificate)

Any member between the ages of 55 – 65 who is retiring from employment may be eligible for Pensioner Membership, provided that they have been a member of Bomaid for an uninterrupted period of 10 years or more. To apply for pensioner membership, please submit the following:

  • Pensioner Membership Application form
  • Letter of confirmation of retirement

Should you need to make amendments to your membership details (e.g. change of surname, banking details, address etc.), please complete a Member Information Update form.

Every Bomaid member is advised to familiarise themselves with their personal benefit schedule and the rules and regulations that govern the society.
